Report:

Each student writes an individual paper documenting some reported accident
in a computer-based system. The report should provide a short account of
the chain of events leading to the accident and an analysis of the causes
of the accident. Any material used, such as reports from an investigation
committee, should be properly referenced and own conclusions made clear.

The report should use the techniques described in the course as a framework
for critical analysis of the case:

how could the accident been avoided (by referring to the course literature)?

which techniques could have revealed the problems (again referring to the
course literature)?

where relevant, a fault-tree drawn using the course software tool should
be used to illustrate the case at an appropriate level of detail.

The report should preferrably be around 3-5 pages long (excluding diagrams
and fault trees).

The deadline for the report is January 10th, 2001.
Presentations:
-
Six of the best handed in reports will be nominated by the examiner
for presentation to the whole class. These students will be presenting
the case at a three hour session (30 minutes each) to be booked around
the end of January.
